The following information will be sought:

     ·    your child's full name

     ·    mother's and father's full names

     ·    occupation(s)

     ·    address of home

     ·    telephone number

     ·    alternative emergency telephone number (nearest relative, friend, or other school parent)

     ·    pre-school experience

     ·    name of family doctor, and permission to contact the doctor in an emergency if a parent cannot be found

     ·    information on vaccinations - vaccination certificate obtained through family doctor;
     serious illnesses or accidents that may raise later problems; any health conditions that need teachers'
     vigilance or response 

     ·    no medication will be given to student unless permission slip is signed by parent.

     ·    ongoing medication – a signed completed form from doctor including name of medication and prescribed dose
     and signed permission slip from parent including medication given to school  

    ·     your child's special interests or activities
